Clinical Care Services

Our clinical care services are currently limited to an area of one hour radius from our Warren, Indiana office. This area includes Bluffton, Fort Wayne, Marion, Muncie, and surrounding locales.

What is a Nurse practitioner? As defined by the American Academy of Nurse Practitioners (AANP), Nurse Practitioners are "advanced practice nurses who provide high-quality healthcare services similar to those of a doctor. NPs diagnose and treat a wide range of health problems. They have a unique approach and stress both care and cure. Besides clinical care, NPs focus on health promotion, disease prevention, health education and counseling. They help patients make wise health and lifestyle choices. They are truly your Partners in Health."

What is a Geriatric Nurse practitioner? The Geriatric Nurse Practitioner (GNP) has received concentrated training in the care and management of the illnesses associated with advanced age. Understanding the disease process and course of treatment subtleties between "adults" and "older adults" allows the GNP to accurately assess and intervene on patient compliance issues, appropriate medication regimens, and the psychosocial needs of the retirement community environment.

How can a NationalHI Nurse Practitioner assist our facility/practice? When you contract with an NP from NationalHI, you can be assured that your patients' healthcare outcomes are the overriding priority. We provide services directly to the patient, billing third party payers, or we can serve as an extension of your existing practice and reassign billing.

When we directly serve residents in your facility as an independent practitioner, there is no direct cost to your organization. We bill Medicare and other payers directly. Through this arrangement, your residents benefit from specialized expertise, staff education, consistent follow-up, single point of contact for routine and acute situations, and more. Generally, the GNP will "collaborate" with your Medical Director to ensure adherence to facility goals and policies.

For the independent medical practice, our GNP services are a valuable extension of your efforts. Subject to regulations, our GNP will visit your patients at the facility -- saving you time while increasing the frequency, and therefore the quality, of healthcare delivery. Billing can be reassigned to your practice, or we can retain the reimbursement minimizing or eliminating any cost to your practice.

When a visit is medically necessary - there simply is no substitution for having a trained provider "lay eyes" on the patient. With documented expertise, the NationalHI GNP offers you success with dementia, pain management, skin and wound care, diabetic management, pessary and other incontinence care, and reduction of polypharmaceutic complications.
